4-2A - food stored covered 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c) Points: 4 Corrected during inspection?: Yes Repeat: Yes Inspector Notes: **First Consecutive Repeat Violation** Observed ice bin in bar area with cover removed. Observed several containers of food in walk-in cooler and reach-in coolers to be stored uncovered while nor actively in cooling state. Food shall be stored in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Corrective Actions: Manager stated cover for ice bin will be placed on ice bin. 4-2B - food-contact surfaces: cleaned & sanitized 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf) Points: 4 Corrected during inspection?: No Repeat: Yes Inspector Notes: **First Consecutive Repeat Violation** Observed old labels stuck to food containers after cleaning. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Note: Manager stated old labels will be cleaned from food containers and new labels will be used instead. 8-2B - toxic substances properly identified, stored, used 511-6-1.07(6)(o) - medicines, restriction & storage (p, pf) Points: 4 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed bottle of Advil stored on shelf with dry ingredients. 1. Except for medicines that are stored or displayed for retail sale, only those medicines that are necessary for the health of employees shall be allowed in a food service establishment. 2. Medicines that are in a food service establishment for the employee’s use shall be labeled with a legible manufacturer’s label and located to prevent the contamination of food, equipment, utensils, linens, and single-service and single-use articles; and located so they are inaccessible to children. Corrective Actions: Manager moved bottle of Advil to proper location during inspection.
