4-2B - food-contact surfaces: cleaned & sanitized 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf) Points: 4 Corrected during inspection?: No Repeat: No Inspector Notes: Observed the can opener to have a thick accumulation of black debris. Equipment, Food-Contact Surfaces, and Utensils. 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC to make sure can opener is thoroughly cleaned before next use. 12B - personal cleanliness 511-6-1.03(5)(i) - hair restraints (c) Points: 3 Corrected during inspection?: No Repeat: No Inspector Notes: Observed employees actively preparing food without proper hair restraint. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. PIC stated that she just took over the store and is waiting for hats to come in. PIC stated that she will order hair nets until the hats/visors come in. 12B - personal cleanliness 511-6-1.03(5)(f) - fingernails (pf) Points: 3 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed employee to have on artificial fingernails while actively prepping food. Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Pf Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. COS: Employee voluntarily washed her hands and placed gloves on during the inspection. 12B - personal cleanliness 511-6-1.03(5)(g) - jewelry (c) Points: 3 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed employee to wearing rings and bracelets while actively preparing food. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Employee voluntarily removed all jewelry during the inspection and wash hands before returning to prep food. 15C - nonfood-contact surfaces clean 511-6-1.05(7)(d) - nonfood-contact surfaces (c) Points: 1 Corrected during inspection?: No Repeat: Yes Inspector Notes: Observed gaskets, located on reach-in cooler underneath the prep-top cooler, to have an accumulation of debris. Also observed nonfood contact surfaces throughout facility to have accumulations of debris (walls, dish washing area). Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC to clean all non-food contact surfaces and implement a cleaning schedule to ensure no accumulation can occur.
