2-2A - management knowledge, responsibilities, reporting 511-6-1.03(2)(m) - person-in-charge duties (pf) Points: 4 Corrected during inspection?: No Repeat: No Inspector Notes: Manager could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. Corrective Actions: Manager stated the employee health agreements stored at a separate location will be brought to the food service establishment for verification of responsibilities to report to management about the 5 symptoms/big 6 diseases. 14A - in-use utensils: properly stored 511-6-1.04(4)(k) - in-use utensils, between-use storage (c) Points: 1 Corrected during inspection?: Yes Repeat: No Inspector Notes: Observed bowls used as scoops for uncooked rice. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon. Corrective Actions: Manager removed bowls and replaced them with scoop with a handle. 14C - single-use/single-service articles: properly stored, used 511-6-1.05(10)(g)(1)&(3) - kitchenware and tableware, single-service/-use items properly handled to protect food-/lip-contact surfaces (c) Points: 1 Corrected during inspection?: Yes Repeat: No Inspector Notes: Unwrapped single-service utensils (spoons) not presented so that only the handles are touched. Single-service and single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-and lip-contact surfaces is prevented. Corrective Actions: Manager rearranged spoons so that handles were facing up. 14C - single-use/single-service articles: properly stored, used 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c) Points: 1 Corrected during inspection?: Yes Repeat: No Inspector Notes: Disposable paper plates are not being kept in the original protective package or are not being stored by using other means that afford protection form contamination until they are used. Single-service and single-use articles shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. Corrective Actions: Manager inverted paper plates so that they are stored to prevent overhead contamination.
